Why Are My Period Cramps So Bad? When Period Pain Is, and Isn't, Normal

Painful periods are common. They are not all normal. That distinction matters more than most people have been told.

What counts as normal period pain

Some cramping during menstruation is a normal physiological response. Mild to moderate cramps in the first one to two days of your period, driven by prostaglandins that help the uterus contract and shed its lining, fall into the category of primary dysmenorrhea. It's uncomfortable, it's real, and it usually responds at least partially to ibuprofen.

When period pain becomes something else

Pain that is searing, burning, or stabbing. Pain that doesn't budge with ibuprofen. Pain that shows up between periods, during sex, or during bowel movements. Pain that makes you miss work, school, or cancel plans regularly. This is a different category, and it often points to something underneath.

Conditions like endometriosis, adenomyosis, fibroids, pelvic congestion syndrome, or pelvic floor tension can all produce severe period pain. Endometriosis alone affects roughly 1 in 10 people with a uterus. The average time from first symptoms to diagnosis is still 7 to 10 years, largely because people were told, repeatedly, that it was just bad cramps.

Your pain is data, not drama

The research is clear: severe menstrual pain that does not respond to basic treatment is not something to push through indefinitely. It warrants investigation. That includes looking at prostaglandin patterns, inflammatory load, hormonal picture, and when indicated, imaging or referral for further assessment.
